Cancer Support

Caregiver Support

A good place to start when you or your loved one is diagnosed with any type of cancer is our Care Navigation service. Our Care Navigators can help you understand the illness and identify resources to support independence as long as possible. Care Navigators will accompany you and/or your loved one to appointments, create a plan of care for the future progression of the illness, and provide caregiver support.

Four Seasons Home Care offers services that help with life’s simple, daily routines and activities. Home Care also provides respite care for loved ones who might need a break.

Enhanced Living

Our Palliative Care Team provides support and symptom management for the physical, emotional, and spiritual issues faced by those living with a cancer diagnosis and their loved ones, allowing for improved quality of life.

Palliative Care is provided by a specially trained team of health care professionals, including a provider, your existing providers, a nurse, social workers, a spiritual care team, music therapists, and others. Our team doesn’t replace your current health care team, we collaborate with your current providers, giving you an extra layer of support, fast response, and help in navigating your care options and the healthcare system.

Ways We Can Help

  • Improved Quality of Life
    • Palliative Care can enhance quality of life for the patient, caregiver, and family.
  • Expert Symptom Management
    • Our highly qualified staff are experts in symptom management, including trouble breathing, insomnia, nausea, loss of appetite, and more.
  • Help Getting Durable Medical Equipment (DME)
    • Our team will work with your primary care provider to order appropriate medical equipment, such as a hospital bed and wheelchairs, if eligible.
  • Fewer Trips to the Hospital
    • We have someone on call 24/7 to triage symptoms by phone in an effort to help prevent the need for frequent emergency room visits.
  • Guidance in Medical Decision-Making
    • We empower the patient, caregiver, and family to better understand and manage cancer symptoms. We also help navigate the healthcare system and make informed decisions about your healthcare choices.
  • Support for Families and Caregivers
    • We spend time with you at each visit discussing the diagnosis, treatment options, and any questions you have. This helps you make informed decisions about what treatments you might want to pursue and if/when it may make sense to stop treatment.

Trusted Comfort

As symptoms progress, Four Seasons can provide trusted support, guidance, and care. We can help you plan for end-of-life care with a focus on comfort and quality of life. We provide Hospice Care in private homes, nursing centers, retirement communities, in the hospital or in our in-patient unit, Elizabeth House. Our Elizabeth House team also provides up to 5 days of Respite Care.

Our experienced Hospice Care team of professional healthcare providers will help your loved one be more comfortable, and make sure you have what you need as a caregiver. We also provide spiritual care, music therapy, counseling services and other support services to help your loved one and you, the caretaker.

We also take care of families and individuals who are grieving the loss of a loved one. Our licensed counselors in Grief Services provide individual and group counseling for children, adolescents, and adults, regardless of hospice involvement.
