At Four Seasons, your assigned Social Worker provides care planning and emotional support to you and your family.
An important resource, they help you identify your primary goals and concerns, and provide useful information about planning your care. In addition, they help with problem solving, accessing community and financial resources and provide support for your caregiver.
Your Social Worker also helps strengthen your coping abilities in times of stress, supports your family and significant relationships and arranges visits with our volunteers.
