Working alongside of our excellent medical providers, our Patient and Family Support Services Team is here to support a variety of physical, emotional and spiritual needs.
The Hospice Medicare benefit was created to address a national growing concern for the need to have more dignity in our end-of-life care. One of the gems associated with this new benefit was the focus on Holistic care for each patient and their designated family/caregivers.
So, what does Holistic Care mean? Within the realm of Hospice, Holistic Care can be defined as: physical, emotion and spiritual support, including support geared specifically toward loss/grief. In order to provide excellent attention to all these dimensions, Four Seasons employees a variety of professionals, all trained to work together in order to provide seamless care.
Our Patient & Family Support Services
At Four Seasons, we believe spirituality is an essential aspect of human experience and a key indicator of quality of life. For this reason, spiritual care support is available for all patients and families connected with our palliative or hospice services. During times of serious illness, terminal diagnosis, or crisis, spiritual care may be the connection that makes all the difference.
Healthcare Social Worker
At Four Seasons, a Healthcare Social Worker helps you identify your primary goals and concerns, strengthens your coping abilities in times of stress, and supports your family. They provide information about care planning, solving problems, community and financial resources, and support for caregivers. Your Social Worker also arranges visits with Four Seasons Volunteers. An essential resource, they are available 24 hours a day, 7 days a week.
Our innovative care includes music therapy, which combines counseling and music to provide emotional and spiritual support to patients and families. Music therapy interventions tailored to the specific needs of each individual patient, family member, and/or bereavement client.
