is ranked as Hospice Honors Elite
The Centers for Medicare and Medicaid Services (CMS) have given Four Seasons a 5 Star Rating in 2026 because of high quality scores on the CAHPS survey. This survey is sent out to families we serve and measures the quality of our communication, timely help, respect, emotional and spiritual support, and symptom management.
In addition, MatrixCare awarded Four Seasons Hospice Hospice Honors Elite Status. The MatrixCare Hospice CAHPS Honors Program identifies high-performing hospice agencies throughout the nation that consistently deliver quality care, as reflected in patient and caregiver experiences on the CAHPS survey.
