Patients living with heart failure may not realize the care and comfort that Palliative Care or Hospice Care can provide. The National Partnership for Hospice Innovation, in collaboration with the American Heart Association, created this Advanced Cardiac Care Program used by Four Seasons as a framework to provide care to patients living with advanced heart disease.
