Care Navigation can help schedule and coordinate healthcare appointments and accompany clients to visits. This ensures that needed appointments are scheduled and kept, and that the right questions are asked at appointments. This further helps ensure treatment plans are followed at home. Care Navigation can also help monitor and plan medications.
Community Resource Coordination
Care Navigation also helps coordinate care and communicate with clients, families, and their providers. Additionally, Care Navigators can connect clients with care resources including in-home support, transportation, social worker support, and more. If you do not live in our area near your loved one, Care Navigators stand in your absence as your loved one’s advocate when you cannot be physically present. This gives you peace of mind that someone who cares for your loved one is involved in their care and will communicate clearly and consistently with you.
Assistance with Life Transitions
Care Navigation helps clients and families with transitions, including downsizing, moving to a new care facility, and more. They can also assist with home safety assessments to ensure your loved one can stay in their home as long as possible.
Planning
Care Navigators are skilled at helping you and your loved one identify what’s most important in their care plans, goals, and wishes. This helps us to help you plan for what comes next, connecting you with the financial and legal resources you may need in preparing an Advance Care Plan. We are your advocate and can help navigate this often complicated and overwhelming process.

Care Navigation is provided through private pay. Funding may be available for those who are eligible through the Four Seasons Foundation.
